Out of state & Kinder:

• Original birth certificate

• Shot record

• Address of school they attended

• One (1) proof of address (power, water, gas)

• NO phone, cable, insurance or credit card statements. NO EXCEPTIONS!

• Parents name must match the name on the bills

• Parent photo ID

• Copy IEP if Applicable
